Patient Portal Info


We're glad you're interested in participating in our patient portal!

You can enroll if you are a patient of a provider within Iredell Physician Network (IPN) or have been a patient at Iredell Memorial Hospital. During a visit at your IPN provider's office, you will be assisted with your portal setup. You will be asked to provide your email address.

You can self-enroll if you have an email address on file with Iredell Health System or your medical record number (MRN). If you still need help, call 704.883.3378.

If you are a patient of an Iredell Physician Network provider, you can use the MyIredell HealtheLife portal to schedule an appointment with FCC Mocksville or another primary care provider in the network. To schedule an appointment with a specialist, please call that provider's office or talk to your primary care provider about seeing the appropriate specialist.

By using the portal, you'll be able to view portions of your medical record online, such as laboratory results, your medication list, hospital discharge instructions and much more information.

You'll also be able to update your health information such as allergies, medications, conditions, etc., and view, print or download a summary of your hospital visit to provide to your doctor.

You may also download the HeatheLife app onto your phone to access your patient portal.

Can't self-enroll? There are three ways to request a portal invitation from Iredell Physician Network.

  1. Online, by clicking here. This is a link to a simple form we will use to email you an invitation.

  2. Calling an Iredell Physician Network representative at 704.883.3378. If you do not have an IPN provider, call the medical records office at Iredell Memorial hospital, 704.878.4589.

  3. Visiting Medical Records at the hospital. (Please note, we do not encourage visits to Medical Records during COVID-19.)

Please note: We stopped uploading patient information to Follow My Health in October 2020. Results uploaded prior to that time will remain in the Follow My Health portal and can continue to be accessed there. Patient information recorded since then will be uploaded to your MyIredell HealtheLife portal if one has been created.

Apple Health

The Apple Health app is connected to Iredell Memorial Hospital’s health record. You must have a HealtheLife account to view your information in Apple Health.

How to add your records via the Apple Health app.

Once enrolled in HealtheLife, you may connect to your health records via Apple Health App.

  1. Open the Apple Health app and tap the Summary tab.
  2. Tap your profile picture in the upper-right corner.
  3. Under Accounts, tap Health Records, then tap Get Started.
  4. Search for your hospital or network, then tap it.
  5. Under Available to Connect, choose an option.
  6. Sign in to your HealtheLife account.
  7. Wait for your records to update. It might take a minute for your information to appear.

After you add an institution, the Apple Health app automatically updates your health records periodically. If you have any questions, please feel free to contact us via email.
