We make every concession to work with your schedule. In the event you must cancel an appointment, please give 24 hours notice to avoid being charged for your missed appointment.

In order for us to provide you with the best possible care, we need the most up-to-date information about you. Please bring the following items to your appointment:

  • Insurance documentation
  • Physician referral form (if required by your insurance)
  • Medication listing including prescription drugs, over-the-counter medications, vitamins and herbal supplements
  • Allergy information
  • Recent ultrasound, radiology and test results (if applicable)

We ask that you present your insurance information at each scheduled appointment. Payment is expected at the time services are rendered by the physician, unless prior alternative arrangements have been made.

Visa, Mastercard, personal checks and cash are all acceptable forms of payment. Unfortunately, at this time, we can not accept American Express or Discover credit cards.

Should you require a prescription refill or have a prescription request prior to your next scheduled visit, please call our office directly. We will make every attempt to accommodate your needs.
